Athletic Forms and Physicals

The NMAA has approved a moratorium for student/athlete physicals.  Parents need to come to the high school office in the afternoon to sign the waiver.  If a student's health has not changed from his/her physical date, on or before April 19, 2019, the student's physical will be considered acceptable for participation.  Transfer students will qualify if their physical from the previous school meets this criteria.  Students with changes or students who did not have a physical in 2019 will need to obtain a physical.  Forms are available at the high school office or Jal Medical Clinic.  All students must have a physical, parent permission forms, and Drug/Alcohol/Tobacco policy permission before being allowed to attend their first practice.  Any questions, call Elaine O'Neal, Principal/AD @ 575/395-2277.
